What has changed? The 2026 Terminology Guide
If you haven’t looked into aged care since 2024, the names have changed. The Australian Government has moved toward a more streamlined system:
Support at Home: As of November 2025, this program replaced the old Home Care Packages (HCP) and Short-Term Restorative Care. It is designed for people who need regular, ongoing, or complex support.
Commonwealth Home Support Programme (CHSP): This remains the “entry-level” option for those who only need one or two basic services (like a weekly cleaner or garden maintenance).
Single Assessment System: You no longer need to navigate different assessment teams. A single assessor now looks at your needs holistically to find the right pathway for you.
Who is eligible for help?
Assistance is available for:
- People aged 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people).
- Those who are still living at home but find daily tasks—like cooking, showering, or getting to appointments—increasingly difficult.
The Three Pathways to Support
Under the new 2026 framework, your help is usually categorised into one of three areas:
- Independence & Everyday Living: Support with cleaning, laundry, home maintenance, and meal preparation.
- Clinical Care: Access to nursing, podiatry, and allied health professionals (like physiotherapists) in your own lounge room.
- Short-Term Pathways: This includes Restorative Care (to help you get back on your feet after a fall or illness) and End-of-Life Care, ensuring you can remain at home with dignity.
How the assessment works now
The process is designed to be “one-and-done” so you don’t have to repeat your story multiple times.
Step 1: Contact My Aged Care. You can do an eligibility check online or over the phone.
Step 2: The Integrated Assessment. A trained assessor visits you at home. In 2026, they use the Integrated Assessment Tool (IAT) to match you with one of 8 specific “Support Classifications” based on your actual needs.
Step 3: Your Support Plan. You receive a Notice of Decision which outlines exactly what funding and services you are approved for.
What are the benefits?
The 2026 reforms brought in fixed price caps and zero clinical out-of-pocket costs for many. This means your funding goes further, and you have more choice over which provider delivers your care.
Whether you need a hand once a week or a coordinated care plan with daily visits, the modern system is flexible. If your needs increase, your “Support Classification” can be adjusted without the long waiting lists of the past.
