Home Care Packages
Stay Independent, Stay Safe
As an approved provider of Government subsidised Home Care Packages (HCP), Kalyra offers a broad range of services to support people to live at home, maintain independence and enhance personal wellbeing through a focus on quality of care. You and your coordinator will develop a personalised and comprehensive support plan, incorporating a range of disciplines and allied health professionals to best suit your needs.
The Home Care Packages program is designed to help people with more complex care needs stay safely in their own homes and retain their independence.
This government-subsidised program provides you with a budget to be spent on agreed upon services. Kalyra’s co-ordinators will help you with practical solutions, so you receive the best service and quality care, while maximising your budget’s reach. You will receive a monthly financial statement to keep track of your budget and our co-ordinators will continue to provide ongoing support.
There is also flexibility in this Program, so you can change the services you receive should your needs or lifestyle goals change.
There are four levels of HCP that can give you the care and services you need:
- Level 1 supports people with basic care needs
- Level 2 supports people with low-level care needs
- Level 3 supports people with intermediate care needs
- Level 4 supports people with high-level care needs
Your Kalyra co-ordinator will regularly assess your program with you to ensure you are happy with the services you are being provided, and will adapt the program if you need to change any services. If your requirements change, and you need additional support, we may be able to help you through this process.
